Double Check Carrier Contact Information When Responding to MG-2s and C-4AUTHs
We recommend all carriers, administrators, and self-insured employers take a moment to ensure the fax and email contact information on file with the Workers’ Compensation Board for responding to MG-2s and C-4AUTHs is accurate.
The Board maintains a database of phone numbers, fax numbers, and email addresses accessible by physicians to determine the appropriate contact information when submitting a request for treatment via either form C-4AUTH or MG-2. The Board’s list contains over 2,600 such fax numbers, organized by W number. Providers simply search the W number and fax the treatment request to the number on file with the Board. Many carriers list multiple fax or email addresses per W number. As such, a provider only needs to pick one of those numbers, and they have satisfied the service requirements of 12 NYCRR 325-1.4. Failure to respond to such a request will result in an Order of the Chair authorizing the requested treatment.
The Board will remove, add, or modify this information only upon your request. Accordingly, we recommend you search the database by your W number(s) and verify that the appropriate fax number or email address is on file. If they are not, you should contact the Board.
